For most people, having “health” is more than just the absence of disease. It also means living with intention and having a sense of purpose in your life. Join us for this one-time class to further explore the concept of Whole Health and what “health” means to you! During our time together you will have the opportunity to complete a personalized health plan, think about meaningful goals, and consider next steps that feel most valuable to your wellness journey. Veterans enrolled with VA Eastern Kansas may qualify for a $200 scholarship to apply toward UFM community enrichment classes when participating in this course.

Eligibility & UFM Community Enrichment Scholarship Opportunities

To be eligible for receipt of a scholarship through UFM, Veterans must show proof of enrollment with VA Eastern Kansas by providing their VA ID to UFM.

Scholarships for community enrichment UFM classes are available to VA Eastern Kansas enrolled Veterans and are available on a first-come basis. Contact UFM for additional details at info@tryufm.org

If a Veteran has not enrolled in VA Eastern Kansas before, enrollment can be done through many avenues. Choose one of the methods below – all will connect you to the enrollment process.

Have the Veteran’s DD214, any private health insurance, and personal information ready prior to starting the enrollment process.

  • Online – Apply For Health Care | Veterans Affairs
    • This method allows Veterans to also set up their VA.Gov Account to apply for other VA benefits.
  • Phone – 877-222-8387 – National Eligibility Call Center
  • By Mail – 1010EZ form– print downloadable PDF and mail to 2200 SW Gage Blvd. Topeka, KS 66622 (enrollment form)
  • In Person – VA Eastern Kansas Eligibility office- 2200 SW Gage Blvd. Topeka, KS 66622

Once a Veteran has completed the enrollment process, the VA will notify the Veteran in one week (or less) if they are eligible for VA health care.  The Veteran can obtain a VA ID card following their enrollment for VA health care. How To Apply For A Veteran ID Card | Veterans Affairs

Classes Not Eligible for Scholarships

Scholarships cannot be applied to any classes that involve alcohol or tobacco.
